Who is eligible to use the KSC Employee Health and Wellness Center? What determines eligibility?
The KSC Health & Wellness Center is available for any employee or dependent covered by the Kokomo School Corporation health plan. Use of the Health & Wellness Center is a win-win for KSC and the employees. The employee or covered dependent receives services in the Center with NO Co-Pay, and the district saves on the cost of services provided in the Center over what is typically paid through the insurance plan.

How much does it cost per visit at the Kokomo School Corporation Health & Wellness Center?
There is NO COST to you.

How far in advance do I need to schedule an appointment?
Appointments are needed, but you can schedule same day appointments, if available, either by calling or by using the online appointment system link located on the schools website: www.kokomo.k12.in.us

Will the doctor share my medical information with the school?
NO! Absolutely not. Your privacy is 100% protected. Services provided are strictly confidential by law under HIPAA. IU Health manages the health records in the same way they are managed throughout the IU Health hospitals and affiliated practices.

My spouse is not on the plan, can they use the facility?
No, the Kokomo School Corporation Health & Wellness Center is only available to those who participate in the Kokomo School Corporation Health Plan.

Will the Center work with my existing doctors if necessary?
Yes, if you receive services in the Health & Wellness Center and want your health care information shared with the doctor or doctors you currently see, with a signed consent/release, the Health & Wellness Center will make sure your doctor receives all information about your visits.

This seems too good to be true. How is it possible?
The program offered costs less than similar care paid for through our insurance. Our goal is to offer you convenient access to high quality care with no co-pay or co-insurance required. Our hope is that by using the Health Center services, you will stay well and feel great. Lower cost and faster-access makes it easier for you to stay healthier. The wellness programs we are offering could potentially help you and the school corporation, avoid higher cost care for more serious illness. It also holds the potential to lessen increases in future insurance costs.

What if I need to see someone when the Health & Wellness Center is closed?
If you have a primary care physician who currently sees you for chronic health condition such as diabetes or hypertension, etc… you will need to continue to follow up with them for these issues & prescription refills.

Does the Kokomo School Corporation Health & Wellness Center offer quality medical care?
Yes. The Kokomo Corporation Health & Wellness Center is fully staffed by a board-certified family practice physician, a registered nurse, and a medical assistant through a partnership with Community Howard Regional Health Network.

Services available at the Health & Wellness Center:
Primary Care: Primary level care to treat & manage acute illnesses Wellness: Health consultation to address any health related concerns or questions.
Labs: Lab services to collect blood & other specimens for analysis & diagnostic needs (including outside lab orders)
Medications: While medications may be available in the clinic; Prescriptions will be written to meet medical needs and referred to the lowest cost vendor. • $4 & $10 retail outlets or other local pharmacy • In clinic medications as available • 90 day maintenance program through Anthem
Preventive: Wellness services include routine annual screenings & physical exams, PAP (well-women) visit, nutrition counseling, health education, and more.
Education: Disease education and routine monitoring of chronic health conditions.
